Diabetes Services

Macon County Public Health offers those suffering from Diabetes a Self-Management Education Program. This program is for adults diagnosed with Type 2 diabetes and their family members or
friends.

Classes are offered several times a year. The program includes:

  • Ten hours of education on healthy eating, being active, checking blood
    sugar, taking medication, problem solving, healthy coping, and
    reducing risk for complications.
  • The cost is covered by many insurance providers. The programs is
    offered on a sliding fee scale for those without insurance.
  • Includes individual meetings with a Registered Dietitian and group
    classes.

How to enroll:

  1. Obtain diagnosis of diabetes
  2. Have your doctor complete referral form on the second page
    of this document (download here in PDF format)
  3. Mail, drop off, or fax referral to Macon County Public Health Center
  4. We will call you to schedule an appointment
